MOBILE VR TRUNK SCHOLARSHIP REQUEST FORM

I certify that the information provided in this Mobile VR Trunk Scholarship application is true and accurate. I agree to comply with the terms and policies of the Mobile VR Trunk Scholarship Program. I understand that if selected, my school will receive a waiver for the cost of borrowing a Mobile VR Trunk in the amount of $500. To thank the Scholarship sponsor(s) and help ensure the continuation of the program, I will send a Letter of Acknowledgement with student feedback to the Museum and have my students complete the online feedback evaluation within 30 days of the end of the Mobile VR Trunk loan.
