Registration forms can be submitted by email, fax (847.967.4839), or mail (9603 Woods Drive, Skokie, IL 60077). If submitting via email, remember to include all information required on the registration form.

Seating is limited and will be filled on a first-come, first-served basis. Maximum two teachers per school.
